Centering Model Overview
Centering® is an evidence-based redesign of health care delivery that helps promote:
Centering is a model of group health care, which incorporates three major components: assessment, education, and support. Patients meet with their care provider and other group participants for an extended period of time, usually 90-120 minutes, at regularly scheduled visits over the course of their care. Centering promotes greater patient engagement, personal empowerment, and community-building.
Requirements for Participation in the Learning Community
To support Michigan Health Centers in building a strong program foundation, the Learning Community will financially support sites in expanding from a traditional prenatal care model to offer group prenatal care. Through a Request for Applications, MPCA will provide start-up funding to cover the costs directly associated with coordination, training, and the purchase of supplies needed for group care. MPCA will also purchase the contract with CHI for the Model Implementation Plan for Michigan Health Centers that have not already received other funding for this purpose through other partners such as March of Dimes.
