A health barrier is the level of challenge of social, personal, environmental, and economic obstacles to a specified health behavior or status.

Health Risk Factors and Outcomes
A health risk is anything which may increase the chance of disease of injury.
A health outcome is a change in the health status od an individual, group, or popoulation which is attributable to a planned intervention or series of interventions.
Michigan Department of Community Health, Vital Statistics
Vital Records are a source of statistical information including births, deaths, marriages, divorces, infant mortality, natality and pregnancy.
County and State Health Statistics Profiles- Profiles of county and state vital events statistics are presented in a one page summary form for each county and the state as a whole. The profiles contain those data most often requested of the Vital Records & Health Statistics Section.
Michigan County Health Rankings
The County Health Rankings are a key component of the Mobilizing Action Toward Community Health (MATCH) project; a collaboration between the Robert Wood Johnson Foundation and the University of Wisconsin Population Health Institute.
Their web site provides a summary rank for each county's health outcomes and health factors and also for the four different types of health factors: health behaviors, clinical care, social and economic factors, and the physical environment. Each county can also drill down to see specific county-level data (as well as state benchmarks) for the measures upon which the rankings are based.

Behavioral Risk Factor Surveillance System
The world's largest, on-going telephone health survey system, tracking health conditions and risk behaviors since 1984 yearly across the United States. Topics included are Alcohol Consumption, Arthritis, Asthma, Cardiovascular Disease, Diabetes, Health Status, Immunizations, Overweight and Obesity and many others. To access their data click here.

Michigan Behavioral Risk Factor Surveillance System
The Michigan Behavioral Risk Factor Surveillance System (MiBRFSS) is composed of annual, state-level telephone surveys of Michigan residents, aged 18 years and older. These annual, state-level surveys, also known as Michigan Behavioral Risk Factor Surveys (MiBRFS) act as the only source of state-specific, population-based estimates of the prevalence of various behaviors, medical conditions, and preventive health care practices among Michigan adults. The annual Michigan surveys follow the CDC telephone survey protocol for the BRFSS and the annual standardized core questionnaire is used. In addition, the Michigan BRFS includes about 25 state-added questions each year.

Right State in Michigan and Its Counties 2011
Right Start is part of the Kids Count in Michigan project. It looks at Michigan births from 2000 to 2009 and ranks counties based on six key measures of health. It has county profiles of counties with over 85,000. See the resource here: Right Start in Michigan and Its Counties 2011.
Michigan Profile for Health Youth
The Michigan Profile for Healthy Youth (MiPHY) is an online student health survey offered by the Michigan Departments of Education and Community Health to support local and regional needs assessment. The MiPHY provides student results on health risk behaviors including substance use, violence, physical activity, nutrition, sexual behavior, and emotional health in grades 7, 9, and 11. The survey also measures risk and protective factors most predictive of alcohol, tobacco, and other drug use and violence. Run your county data here.
DHHS, Community Health Status Indictors
The goal of Community Health Status Indicators (CHSI) is to provide an overview of key health indicators for local communities and to encourage dialogue about actions that can be taken to improve a community’s health. The CHSI report contains over 200 measures for each of the 3,141 United States counties.The CHSI report provides a tool for community advocates to see, react, and act upon creating a healthy community. The report can serve as a starting point for community assessment of needs, quantification of vulnerable populations, and measurement of preventable diseases, disabilities, and deaths. Uniform Data System
The Uniform Data System (UDS) tracks a variety of information, including patient demographics, services provided, staffing, clinical indicators, utilization rates, costs, and revenues. UDS data are collected at the grantee (individual CHC), state, and national levels. It is released annually in late summer.
The data is reviewed to ensure compliance with legislative and regulatory requirements, improve health center performance and operations, and report overall program accomplishments. The data helps to identify trends over time, establish or expand targeted programs and identify effective services and interventions to improve the health of underserved communities and vulnerable populations. .

UDS Mapper
HRSA, John Snow, Inc. and the Robert Graham Center have collaborated to develop a mapping and decision-support tool driven primarily from data within the Uniform Data System (UDS). The UDS Mapper is designed to help inform users about the current geographic extent of U.S. federally (Section 330)-funded health centers. You have to register for access, but it is open to all users. Click here to do so.
Health Resources and Services Administration Data and Statistics
The Health Resources and Services Administration (HRSA) is an Agency within the U.S. Department of Health and Human Services. As the Nation’s Access Agency, HRSA focuses on uninsured, underserved, and special needs populations. HRSA Data provides access points to current HRSA information, health resources, and demographic data for reporting on HRSA activities. HRSA produced in 2010 a document to assist health centers in documenting need in their community and/or for their target populations called Data Resources for Demonstrating Need for Primary Care Services. The data resources focus on barriers to care, health indicators, and disparities.
