Community Healthcorps

Michigan Primary Care Association Community HealthCorps members are committed to  "Serving Our Communities by Building a Healthier Michigan through Patient-Centered, Community-Focused Navigation.“  As part of Michigan's AmeriCorps program, each member serves a one-year term at one of our partner sites across the state in order to improve the health of Michigan residents by serving as:Americorps Logo
  • Patient Navigators, support and mentor individuals struggling with chronic health conditions, support the development of a Patient-Centered Medical Home, and help in the development and integration of a skilled-volunteer base to strengthen the support offered to patients.
  • Community Navigators, seek to address health disparities prevalent in the community through health education and outreach efforts (including enrollment initiatives), support the development of new collaborative partnerships to address community health, and help in the development and integration of a skilled-volunteer base to strengthen the promotion of community health.

Key Components of MPCA Community HealthCorps

The MPCA Community HealthCorps program is committed to providing members and the communities they serve with the four key components of a successful National Service experience:

In addition, our members support the seven characteristics of a Patient-Centered Medical Home:
  • Help Patients Establish and Maintain a Personal Relationship with their Healthcare Providers
  • Be Part of a Team Approach to Healthcare that Includes the Patient
  • Support a Whole-Person Approach
  • Assist with the Coordination and Integration of Care
  • Promote Quality and Safety
  • Expand Access to Care
  • Add Value for Patients and in their Community

Member Benefits

The following benefits are made available to MPCA Community HealthCorps members:
  • Living Allowance: Members will receive a living stipend of $12,000 during their first term of service.
  • Education Award: Upon successful completion of their term of service, members will receive an education award of $5,350 from the National Service Trust.
  • Health Insurance: Full-time members are eligible to receive basic health insurance.  The coverage is made available at no cost to the members.
  • Child Care: Any AmeriCorps member who is a legal parent or guardian, residing with the child, may be eligible for childcare assistance.
