Overview


Join the Michigan Primary Care Association for a full-day workshop designed to highlight what daily continued practice transformation looks like in all areas of your organization. Nationally recognized consultants from Coleman Associates and the Iowa Chronic Care Consortium will be presenting high-quality, evidence-based materials in support of this concept. 

 

Health center executives (e.g., CEO, COO, CFO, or CIO), quality improvement, information technology, and all clinical staff are welcome!

 


Agenda


This workshop will consist of two general sessions.

 

The morning general session will be presented by Coleman Associates. Coleman Associates will address some level setting conversations around Value Based Payment and its implication for Health Center administration, leadership, operations and patient and staff engagement. They will preview the NACHC readiness assessment tool and discuss some of those key areas with an emphasis on the areas of team care and coordination. Later, Coleman Associates will share how to identify, address and work through resistance to change and how to best bring staff and colleagues along a journey of change. The session will wrap up with some next steps around leadership activities and communication to get the ball rolling on care coordination as a tangible and necessary step along the evolution toward value-based payment.

 

The afternoon general session will be presented by Iowa Chronic Care Consortium. This session will identify needs related to clinical communication and care delivery by:

  • Designing a team-based care model that improves efficiency and invites innovative team roles in addressing the unique and personalized care needs for patients 
  • Developing a population-based approach to identifying high risk patients and delivering appropriate interventions
  • Creating an engaging environment that is welcoming to patients, supportive of their holistic health interests and prepared to address a wide range of social determinants
  • Building the case for a new kind of care encounter that inspires patients toward improved self-management and strong (therapeutic?) patient-provider relationship
  • Maximizing new payment opportunities offered through state Medicaid agencies, commercial value-based contracts and Medicare.  Most involve a foundation of proactive care management and care coordination for high risk patients
