This event is the final Learning Session of the MPCA PCMH Learning Community. Each health center that is a member of the Learning Community will get one free registration. This event is designed for all health center staff managing projects including transformation and information management.

This event is designed for all health center staff managing projects including transformation and information management.

Description:

Health Centers need some form of "organizational endurance training” to meet and sustain the increasing challenges of multiple initiatives. The Triple Play takes advantage of the alignments among multiple initiatives, applying the data and keeping it all going in an environment that demands continuous change.

This one day interactive event proposes to community health centers a framework that; integrates the key leverage points and commonalities between multiple initiatives (e.g. PCMH, MU, UDS), applies data to decisions, documentation, practice redesign and compliance, and builds some practical methods to sustaining the changes made during the compliance process.

Peter Cucchiara and his team provide principles, practices, examples, dashboards and models with the intent of stimulating thinking around policy, process, performance and outcomes. The methods and perspectives presented during the interactive learning lab suggest an integrated interactive approach to building a framework for making a triple play strategy successful.

Registration Fee:

Free - for the first PCMH Learning Community Member

$ 75 - for the second or more PCMH Learning Community Members
$ 75 - for MPCA Health Centers not participating in MPCA's PCMH Learning Team

Hotel Group Rate: $99 + $5 hotel fee, state and local taxes

Reservations can be made by calling the Kellogg Center at 1.800.875.5090. Be sure to let them know you are with the Michigan Primary Care Association room block. The hotel cannot guarantee the group rate for reservations after April 30, 2014. Reservations made after this date will be subject to room availability.
