MI Care Team (2703 Health Homes) is off to a strong start. Ten Michigan Health Centers have committed to serve a population of approximately 18,000 through an innovation model of care with access to never before available technology and data through Care Connect 360. In partnership with Medicaid Services Administration, Michigan Primary Care Association is coordinating a comprehensive MI Care Team Learning Community that includes training, technical assistance, and peer networks. It is imperative to your success that all Health Center participates and engage fully in the Learning Community.
The Health Homes Coordinators Meeting/Training will be held on May 10th at Treetops Resort. All Coordinators are expected to attend and for this meeting we will allow one additional support person. This meeting will begin at 10 AM and conclude at 4:30 pm. Lunch and a 5:30 reception dinner will be included. (Cash bar)
The required face-to-face Orientation and Training for MI Care Teams is May 11th at Treetops Resort in Gaylord Michigan. Health Centers are expected to send key leadership staff representing medical, behavioral health, quality management, care management , information technology finance, billing, enrollment, care coordination, community health workers and referral managers. This meeting will begin at 8:30 am and end around 4:30 pm. A continental breakfast and lunch will be included.
The Learning Community contract will allow financial support for a majority of the expenses for this event to include the following:
- The training - There will be no registration fee, but advanced registration is required.
- Coordinators (plus 1) from each Health Center – Cost of overnight accommodations; limited to two nights; May 9th and 10th
- Hotel cost for up to five (5) people from each Health Center – 100% of the overnight hotel room cost for one night will be covered. The first five people must provide their hotel information on the registration event page when registering. Shannon Lijewski with MPCA, will make your reservations for you and send you the hotel confirmation number. PLEASE NOTE: Any "no shows" or those where cancellations were not received before the cancellation deadline will be invoiced the cost of the hotel room. See TreeTops cancellation policy below.

For anyone registering more than five people per Health Center, please make your reservations directly with Treetops by calling 1-855-261-8764, and ask for the Michigan Primary Care Association room block. The deadline to reserve your hotel rooms before the discounted room block is released to the general public is April 21, 2016.
Treetops Hotel Cancellation Policy
Deposits are refundable if notice of cancellation is received by the Resort Reservations Department at least seven (7) days prior to arrival.
