Webinar Overview

Our sister Primary Care Association in Wisconsin is sharing best practices on referral management. Join us for a review on Care Coordination relative to requirements and recommendations for sharing patient health information for referrals.

  

Attendees will be given a review on Care Coordination and how a coordinated approach could improve referral management and tracking, staff collaboration, and better patient outcomes. They will be shown resources and tools they can utilize in their Health Centers to create a better referral process, including quality content in referrals and care coordination agreements. Attendees will also learn about requirements and recommendations for legal medical record, designated record set, continuing of care requests for sharing patient health information for referrals and patient privacy basics.

  

Session Learning Objectives

  1. Better understand how Care Coordination can enhance referral management at their Health Center.
  2. Obtain at least one specialty resource or tool that can be accessed to help improve the referral process at their Health Center.
  3. Discuss operational strategies and legal requirements for sharing patient health information for referrals.
  4. Increase knowledge of HIPAA guidelines for patient referrals.
  5. Gain strategic ideas and tactical skills to use on the job.
