Position Summary
American Indian Health and Family Services of Southeastern Michigan - Minobinmaadziwin (AIHFS) is a non-profit ambulatory health center, founded 1978. AIHFS provides quality, culturally integrated, medical, preventative dental, in addition to comprehensive diabetes prevention and treatment and behavioral health services. AIHFS also provides youth and their families a positive support network that focuses on physical, mental, emotional and spiritual balance. All of AIHFS programs integrate traditional Native American healing and spiritual practices with contemporary western medicine in both treatment and prevention.

Under general supervision, the Social Worker provides advice and counsel to patients and families. Helps patients and families access assistance programs, including for insurance and meeting basic needs. Makes and tracks referrals for therapy and other services as appropriate. Provides guidance to medical personnel regarding the impact of psychosocial factors on the treatment and progress of patients. Assess and develops treatment plans for the client and or their family.

Qualifications

  • MSW required with 3 - 5 years' clinical experience, preferably
  • Must be licensed in the State of Michigan and certified to practice as a certified clinical social worker as required by the State of Michigan Social Work Certification Board
  • Thorough knowledge of social work principles, techniques and practices and their application to complex casework, group work, and community problems
  • Considerable knowledge of a wide range of behavior and psychosocial problems and their diagnosis and treatment using the DSMIV
  • Considerable knowledge of family and group dynamics, a range of intervention techniques, governmental and private organizations, resources in the community, laws, regulations and policies which govern the program, methods and principles of casework supervision and training
  • Ability to supervise, train or instruct low level social workers, students or interns in the program, ability to establish and maintain effective working relationships with members of caseload and their families as well as civic, legal, medical, social and religious organizations
  • Experience working with Native children and families preferred
  • Experience working with Substance Abuse prevention and treatment on a one-to-one basis and in group settings, a plus
  • Proficient in Microsoft Office Suite

Under direction of the Director of Clinical Services, plans, organizes and coordinates HIV project activities to ensure the goals and objectives of
the project are achieved by efficient and effective utilization of resources. Coordinates activities by scheduling work assignments, setting priorities, and directing the work of the HIV team members. Identifies staff development and training needs and ensures that training is obtained. Coordinates outreach activities including monitoring staff, problem solving and evaluating outcomes. Ensures that an effective patient referral and tracking system is in place. Manages areas of responsibility within the established health center budget. Develops and implements policies and procedures to improve the quality and quantity of work. Attends required meetings and participates in committees as requested. Monitors patient satisfaction and responds to patients’ complaints in a timely manner. Assures compliance with HIV grant guidance and reporting timelines. Accurately prepares and supervises the compilation of all month end closing procedures and reports. Coordinates all ancillary services i.e. laboratory, nursing, and specialty clinics. Assures adequate HIV testing and other supplies are maintained. Maintains a professional demeanor and effective communications at all times. Performs other related duties as required.

Qualifications

  • Knowledge of the principles and practice of public health and of health-care delivery.
  • Knowledge of the state and federal laws pertaining to health-care delivery and to federally qualified community health centers.
  • Knowledge of the structure and function of community health centers.
  • Skill in communicating effectively with the staff, patients and their families.
  • Skill in establishing and maintaining effective working relationships with staff, supervisors and colleagues.
  • Ability to multitask and solve problems while maintaining a calm demeanor.
  • Proven leadership ability to effectively direct staff and the ability to follow directives from others.
  • Graduation from an accredited college or nursing program.
  • A minimum of two years progressively responsible community health nursing experience, excellent interpersonal, oral and written communication skills.

AIHFS is seeking a full time Primary Care and Family Practice Physician. This involves the delivery of family oriented medical care services as well as provides general professional guidance of primary care staff.

Qualifications

  • Ability to motivate, train and work effectively with subordinates who have a variety of backgrounds and training
  • Ability to accomplish the quality and quantity of work expected within set limits of cost and time
  • Ability to plan own work and carry out assignments effectively
  • Ability to communicate with others effectively both orally and in writing in working out solutions to problems or questions relating to work
  • Ability to develop improvements in or design new work methods and procedures
  • Ability to establish rapport and gain the confidence of others (physicians, nurses, members of the primary care team, patients and associates)
  • Ability to function as a health care delivery team member
  • Ability to assess quality patient care
  • Ability to maintain patient medical records
  • Skills in providing diagnostic, preventive, and/or therapeutic services to patients
  • Must be able to work with confidential material without betraying the trust of patients, families, or the agency
  • Completion of an accredited medical school, internship, and completion of the certification examination by the medical board of examiners
  • A permanent current full and unrestricted license to practice medicine or osteopathy in Michigan
  • Board certified or eligible in family practice
  • If Board eligible, must be AAFP or ACOFP Certified within six months from the date of hire
  • Current medication dispensing license (DEA)
  • Experience and training must have been progressive and responsible, demonstrating good knowledge of current principles, practices, methods, and techniques in the field of medicine
  • Medical experience in an outpatient family medical clinic including pediatrics, obstetrical/gynecological, medical care, and non-emergency care
  • Possess current and valid Michigan Driver's License with no DUI/DWI or reckless driving convictions in the last five years having no more than two at-fault accidents in the last three years and maintain a valid Driver License during employment
  • Must pass a criminal background check with a Class I Fingerprint Clearance Card within the initial ninety (90) days of employment
  • Familiarity and experience working with the Native American Community at a local, state and national level
  • Respect for and knowledge of traditional, cultural and spiritual practices of a diverse Native American community, as well as ability to work with other racially, culturally, and ethnically diverse populations
