Traverse Health Clinic Partner in Community Health Needs Assessment Survey

As required by the Affordable Care Act, non-profit hospitals must complete a Community Health Needs Assessment every three years.
For the 2016 assessment, Munsion Healthcare in Traverse City, Michigan is once again partnering with other health care providers in the area including the county health departments, school superintendents, community members and the Traverse Health Clinic.
Pressing health issues identified during the 2013 survey included a high percentage of maternal smoking, obesity, diabetes, access to care and medication. To help address the identified issues, community partners developed a plan that includes the creation of a website to providing health living resources, increased diabetes education, and helped patients find care and medicine at a reduced or no cost.
