You are invited to participate in the Sponsorship & Exhibitor Program for the 2014 Putting Together the Pieces for Comprehensive Care conference to be held on Wednesday, October 15, 2014, at the Radisson Plaza Hotel & Suites in Kalamazoo, Michigan. We anticipate an interdisciplinary audience of approximately 150 participants including Federally Qualified Health Center (FQHC) staff, Community Mental Health (CMH) staff, and other safety net providers from around the state.

The goal of the Sponsorship & Exhibitor Program is to enhance the overall quality of the conference, and to inform participants of goods and services available to strenghten their organization and outcome measures. Your sponsorship and/or exhibit will be greatly appreciated throughout the event.

Sponsorship & Exhibitor Opportunities

The exhibit area will be visited frequently throughout the day during registration, meals, and refreshment breaks, guaranteeing networking time with conference attendees. The day will feature at least two breaks and will allow time for attendees to move throughout the room as they transition from general sessions to breakout sessions. Sponsors and exhibitors will be recognized in the official on-site materials and announced during the conference. The following opportunities are available:

Lunch Sponsor: $2,000

  • 8-foot display table
  • Complimentary conference registration for up to 2 exhibitor representatives

General Session Sponsor: $1,500

  • 8-foot display table
  • Complimentary conference registration for 1 exhibitor representative

Breakfast Sponsor: $1,000

  • 8-foot display table
  • Complementary conference registration for 1 exhibitor representative

Breakout Session Sponsor: $1,000

  • 8-foot display table
  • Complimentary conference registration for 1 exhibitor representative

Refreshment Break Sponsor: $750

  • 8-foot display table

General Sponsor: $500 minimum

  • 8-foot display table

Door Prize Donation

A drawing for door prizes will take place at the closing of the general session. Please indicate on the registration form below if you would like to donate an item for the drawing and give a brief description of that item. If you are donating an item, please bring the donation to the conference registration desk on October 15, or ship it to MPCA by October 1, 2014 (MPCA, Attn: Brittany Pazdan, 7215 Westshire Drive, Lansing, MI 48917).

The completed registration form and payment are due on or before Monday, September 15, 2014.
