CHW Certificate Scholarship Program

The Michigan Primary Care Association (MPCA), in an effort to encourage continuing education and Community Health Worker (CHW) Certification in Michigan health centers, will be offering scholarships to member health centers to support the future growth of the CHW role in providing patient enabling services and community linked supports. This is a reimbursement program.

 

MPCA is awarding $1,000 scholarships per CHW to financially support the certificate program for CHWs attending, or have attended, a Michigan Community Health Worker Alliance (MiCHWA) sponsored CHW Certificate program.

 

This scholarship opportunity is competitive and funds are limited. Scholarship applications will be awarded based on the completion of the application, letter of recommendation from the CHWs direct supervisor, and details provided within the CHW's 300 - 500 word personal essay. Scholarship preference will be given to CHWs currently working on MPCA supported programs and initiatives.

 

 

All applications must be submitted to MPCA by September 1, 2017 and CHWs must complete certification training on or before December 8,2017.

 

 

Overview

The Michigan Primary Care Association (MPCA) established the Community Health Worker Scholarship to assist member health centers in financially supporting current Community Health Workers (CHW) who have not participated in a Michigan Community Health Worker Alliance sponsored CHW Certificate Program. MPCA will reimburse participating health centers with the $1000 scholarship to cover the cost of training, or travel expenses upon the receipt of a photocopy of the CHW's training certificate of completion.

 

 

Eligibility

To be eligible for a Community Health Worker Certificate Program Scholarship, a candidate must

  • Be employed by an MPCA member health center.
  • Complete the Scholarship application in its entirety.
  • Be a current Community Health Worker and/or health center employee acting or providing support to patients in a CHW capacity.
  • Submit a letter of recommendation from a direct supervisor and a personal essay highlighting the current CHW position and job responsibilities.
  • Be committed to working in primary care in under served areas in the community.
  • NOTE: CHWs who have completed a certificate program between January - May 2017, or are currently enrolled in an approved CHW certificate program are eligible to apply. 
  • NOTE: Failure to follow all these instructions may be cause for rejecting your application

 

Submission Requirements

  • Letter of recommendation from a direct supervisor.
  • Professional resume.
  • Up-to-date job description.
  • 300-500 word personal essay

 

Personal Essay Guidelines

 

Please address the following questions in your personal essay:

  •  Describe your educational and career goals.
  •  Describe how your role as a CHW  has impacted the health center and patients you serve.
  •  How will a CHW certification benefit your ability to perform your responsibilities and duties?

 

 

Terms and Conditions

Grantees will receive scholarship payment upon completion of the MiCHWA sponsored CHW certificate program. I understand that I must submit a photo copy of a training certificate no later than December 8, 2017 in order to receive the financial award.

 

By submitting my application for the MPCA Community Health Worker Certificate Program Scholarship, I certify that the information provided in this application is complete and correct to the best of my knowledge, and I will notify MPCA of any charges to the information provided.

 

By submitting an application, I grant MPCA permission to use my application material for promotional purposes and agree to participate in promotional activities, as requested by the Association and/or scholarship sponsors.

 

MPCA's Community Health Worker Certificate  Program Scholarship is funded by the Michigan Health Endowment Fund.
