MPCA Hosting CHW Training

The MI Care Team (2703 Health Homes) is off to a strong start! Ten Michigan Health Centers have committed to serve a population of approximately 18,000 through an innovative model of care. 

In partnership with Medicaid Services Administration, the Michigan Primary Care Association is coordinating a comprehensive MI Care Team Learning Community that includes training, technical assistance and peer networks. It is imperative to the success of MI Care Team for all participating Health Centers to fully engage in the Learning Community.

The MI Care Team Community Health Worker (CHW) Training will be held on June 22, June 23 and June 24 at the Bavarian Inn in Frankenmuth. All MI Care Team CHWs are expected to attend. 

June 22, 2016
Training: 12 pm ET - 4 pm ET, lunch will be provided
Audience: MI Care Team (Health Homes) CHWs

June 23, 2016
Registration and breakfast: 8:30 am ET; Training 9 am ET - 4 pm ET
Audience: CHWs participating in MI Care Team, Cancer and Linkages

June 24, 2016
Training: 8:30 am ET - 4:30 pm ET, continental breakfast and lunch will be provided
Audience: MI Care Team Health Homes Coordinators, MI Care Team CHW Supervisors, Cancer CHW Supervisors and Linkages CHW Supervisors

MDHHS Chief Deputy Director Departing

On May 31, 2016, the Michigan Department of Health & Human Services announced Chief Deputy Director Tim Becker is leaving the Department for a new position with Hope Network.

Tim's last day will be July 8 and a new Chief Deputy Director will be announced in the coming weeks. In his new role, Tim will serve as the executive Vice President with the Hope Network.

The second communication focused webinar, Growing Your Advocacy Network during National Health Center Week & Measuring the Results, is scheduled for June 22, 2016 from 2 - 3 pm ET. The webinar will be presented by the National Association of Community Health Centers Advocacy and Communications team and will focus on growing your advocacy network during National Health Center Week, scheduled for August 7 - August 13, 2016.

Undiagnosed Hypertension in the Safety Net Webinar Scheduled

With the shift to value-based care, managing chronic diseases effectively is critical. Health Centers will need to make sure they are treating all patients with elevated blood pressure including patients with undiagnosed hypertension who are "hiding in plain sight."

Health Centers Serve & Hire Growing Number of Veterans

More than 300,000 veterans are receiving health care services at Health Centers, according to the National Association of Community Health Centers (NACHC). And the number of veterans seeking care at Health Centers is expected to continue to grow as a result of the federal law that created the Veterans Choice Program (VCP).  

The VCP, which is the centerpiece of the Veterans Health Administration’s (VHA) efforts to expand its coordinated community care programs, allows Health Centers to open their doors to military veterans across the country in an effort to reduce excessive waiting times at VHA facilities. 

NACHC has produced a fact sheet about the program, including Frequently Asked Questions about how the program works, and who is eligible.
