Health Centers have a critical role in patient and community education, outreach, and health navigation/coordination. There are a multitude of current and upcoming opportunities for Health Centers to expand services beyond enrollment, and begin providing community-based service linkages. Michigan Health Center staff have the knowledge and skills needed to help patients and communities address barriers and social determinants of health. MPCA, in turn, assists Health Center staff through technical assistance.

Enabling Services Annual Forum: Connecting Consumers to Care and Community
The MPCA Enabling Services team is hosting our annual forum on October 3rd, at the Crowne Plaza Lansing. This year’s forum will offer networking opportunities for a variety of health center staff, including Outreach and Enrollment Specialists, Community Health Workers, Navigators and more! The day will begin with a general session on building strategic partnerships, presented by Kristen Stoimenoff Deputy Director of Health Outreach Partners. The day will also feature sessions on social media outreach, MDHHS appeals and integrated service delivery, enrollment assister 101, and targeted outreach. See below for a list of our full outstanding speaker line-up!
*FYI* This year's Enabling Services Forum will replace our annual Outreach and Enrollment (O/E) event; however O/E sessions will be incorporated in the days agenda and all sessions will provide information and resources applicable to an O/E program. The decision to broaden our focus and include health center staff beyond O/E, aligns with our plans to support health centers in identifying and addressing all patient social determinants of health.

Michigan Primary Care Association’s (MPCA) federal grant program titled "State and Regional Primary Care Associations” allows MPCA to provide technical assistance and support to Health Centers in Michigan and is 100 percent funded by federal dollars at a current annual level of $1,069,988.00.
