Health centers have a critical role in patient and community education, outreach, and health navigation/coordination. There are a multitude of current and upcoming opportunities for health centers to expand or enhance services providing community-based linkages. Michigan health center staff have the knowledge and skills needed to help patients and communities address barriers and social determinants of health. Michigan Primary Care Association (MPCA), in turn, assists health center staff through technical assistance, training, peer communication network facilitation, resource development, and best practices sharing.

Apply for the MPCA Community Health Worker Certificate Program Scholarship!


The Michigan Primary Care Association (MPCA) established the Community Health Worker Certificate Program Scholarship to assist member health centers with financially supporting current Community Health Workers (CHW) who have attending or will be attending a Michigan Community Health Worker Alliance sponsored CHW Certificate Program in 2017. The scholarship is intended for the cost of training or travel expenses which will be reimbursed after successful completion of training and receipt of the certification of completion. Scholarship deadline is September 1, 2017.  Please click here for more scholarship details!
