Linking Clinical Care with Community Supports (Linkages)
Michigan Primary Care Association (MPCA) was awarded funds through the Michigan Health Endowment Fund for the Linking Clinical Care with Community Supports (Linkages) Project; to hire, train, and integrate Community Health Workers (CHWs) into primary care teams in 16 Health Centers throughout Michigan. This project directly supports patients by sharing information and resources for accessing food, housing, social services, and counseling. Linkages CHWs will also monitor patients' health and help increase accountability through individualized patient plans.
Who We Serve
Each CHW works within a Care Team and focuses on one of three populations.
CHW and Care Team Training
Twenty nine CHWs completed the third pilot training program provided by Henry Ford Community College and the Institute for Population Health for CHW certification. The training program comprised of 126 classroom hours and 40 internship hours. The curriculum emphasized the care competencies including patient advocacy and outreach, personal health strategies, capacity building, ethics, care coordination,documentation and reporting, cultural competence and health promotion.
MPCA provided additional training focused on developing an understanding of team-based care as well as skill building in areas of documentation of patient encounters and motivational interviewing. Training was also provided on Diabetes, Hypertension, Asthma, Pregnancy, Obesity, and Depression. During two in person trainings, Care Teams were given an opportunity to learn workflow redesign, best practices on team communication, and team dynamics to help them adjust to their new Care Team.
To individually support the training provided, MPCA continues to host biweekly one on one coaching calls with CHW's and monthly coaching calls with Linkages Health Center Care Team lead staff. During these calls, teams receive data driven feedback for rapid improvement of their individual team needs. Linkages staff has also conducted onsite visits with each participating Health Center to work with teams individually to overcome challenges and share best practices.
