Membership Application - Active or Associate

If your organization is interested in applying for Active Membership or Associate Membership in Michigan Primary Care Association, please complete and submit this form. (After clicking the submit button at the bottom of the form, a confirmation webpage should appear. If it does not, that means your application was not received by MPCA and you should email info@mpca.net to let us know.)


1. Active Membership is open to providers of primary care, and comprehensive, community-based organizations, including organizations and individuals that support MPCA's mission. Active membership is not available to individuals who are employed by an organization eligible for membership as an organization.

Active Membership benefits:
Employees and board members of Active Member Organizations enjoy training and technical assistance, participation in MPCA-hosted Peer Communications Networks, free or discounted registration to MPCA-hosted trainings and events, online resources, subscription to the weekly MPCA eUpdate, and much more.

Active Membership Annual Dues: Based on annual operating budget.


2. Associate Membership is open to individuals or organizations that do not provide primary care and support the mission of MPCA.

Associate Membership benefits:
Employees of Associate Member Organizations enjoy free or discounted registration to MPCA-hosted trainings and events, online resources, subscription to the weekly MPCA eUpdate, and much more.

Associate Membership Annual Dues: $2,500
