Value-based care requires effective population health management to succeed. For that to happen, proper data integration, maximization of existing resources, and care management strategies are necessary.


MPCA’s fall conference, Intersection of Population and Public Health, can help. Join us from November 7 to 8, at the Crowne Plaza Hotel in Grand Rapids, Michigan, for a two-day conference designed to give you the tools you need to succeed in a health care environment that increasingly favors value-based care.

  • Wednesday, November 7, will include a Care Manager Network face-to-face meeting followed by a day of dynamic training focused on health care team infrastructure, maintaining clinic flow and time management, and the financial case for nursing support. You’ll also learn how to proactively manage chronic diseases, including diabetes and hypertension, through collaboration and innovation.
  • Thursday, November 8, will focus on population health and how it intersects with the work you do at community health centers.

  • Afternoon session  |  Azara Statewide Quality Forum. 

Day 2 - Nov 8 - Includes three general session, as well as three tracks; 1) Care Management, Health Innovations, and Value Based Payments
