Strategies to Improve Health Center Performance in a Value-Based Environment – Care Coordination

Care Coordination plays an essential role in building and supporting high functioning care teams, and the delivery of comprehensive patient centered care. Many health centers use the terms Care Management and Care Coordination interchangeably, when in fact, they are two distinctly different services with unique goals and objectives. Participants will learn: Define and differentiate Care Coordination from Care Management. The focus, goals and objectives of Care Coordination. The role of Care Coordination in Population Health Management.
