Overview of the Fall Clinical Conference

Although many health outcomes are determined by social determinants, few clinicians can ably identify those patients facing challenges related to social and environmental conditions or other experiences that directly impact health and health status. It is important to bridge the clinical and social services to achieve the desired health outcomes. The care teams can begin to identify and address social determinants, dramatically impacting patient outcomes as well as their own financial success under value-based care.

 

For that to happen, proper data integration, maximization of existing resources, and care management strategies are necessary.

MPCA’s fall conference, The Role of Social Determinants in Clinical Outcomes, will exhibit this partnership of clinical and social services. Join us from Oct. 7 to 8 at the DoubleTree Hotel in Bay City, Michigan, for a one-and-a-half day conference designed to give you the tools you need to succeed in a health care environment that increasingly favors value-based care and improved patient outcomes.


Overview of Enabling Services Forum 2019

Enabling services provide support that enable individuals to access health care and improve health outcomes in order to supplement clinical care. These services can include case management, referrals, translation/interpretation, transportation, eligibility assistance, health education, environmental health risk reduction, health literacy, and outreach.

Agenda

Day 1 will focus on social determinants and their impact on chronic disease. To view the draft agenda for day one, please click here

 

Day 2 will consist of the following two trainings.

  • NCQA PCMH Recognition Annual Reporting Training is designed to help PCMH practices understand the larger goals of annual reporting and prepare for submission under the redesigned PCMH program. NCQA faculty will demonstrate sustained recognition status through continuous quality improvement efforts and the use of already collected data. Timelines for deliverables and performance expectations will be key discussion items.
  • Cultural Humility Training (4 hours) will be geared towards enabling services staff and is limited to the first 20 registrants.

Registration

Registration fees include a continental breakfast and lunch.

  • Monday, Oct. 7, only 
    $125 - members
    $150 - non-members
  • Tuesday, Oct. 8, only (NCQA and Cultural Humility Training)
    $125 - MPCA members only

The NCQA Training is limited to two (2) members per health center. Each health center will work with Simmi Isaac to determine who is attending. Those attending will be registered by MPCA and invoiced for the $125 registration fee. To register for the Cultural Humility Training, email Nashia Choudhury. 

Cancellations and refunds must be submitted in writing. Substitutions are accepted in lieu of cancellations. A 10% administrative fee is charged for all cancellation and refund requests. No refunds will be given within 5 business days of the event. No refunds will be given to "no shows".

Hotel reservations can by made by calling the hotel directly at 989.891.6000 or 800.222.8733 by Sept. 15, in order to received the discounted group rate. Be sure to ask for the Michigan Primary Care Association room block. The DoubleTree cannot guarantee the discounted rate after Sept.15, and any reservations made after this date will be based on a space-available basis.

 

Hotel cancellation policy: All reservations must be cancelled by 6 p.m. on the day prior to your arrival date to avoid a cancellation fee equal to one night's room and tax.
