DESCRIPTION:\n\nOverview\nPlease mark your calendars for November 2nd and 3
 rd\, 2023.  The conference will take place at the Crowne Plaza West Lansi
 ng.  This two-day in-person event will focus on initiatives\, tools and b
 est practices that support equitable and sustainable population health man
 agement programs in Michigan Community Health Centers. \n\n\n\nWho should
  attend?\n\n 	Clinicians (MD\, DO\, NP\, PA\, DDS\, PharmD) \n 	Clinical 
 Leadership (CMOs\, CDOs\, Clinic Directors) \n 	Quality Improvement Staff
  \n 	Clinical and Non-clinical Support Staff (Care Managers. Medical Assi
 stants\, CHWs\, Nurses\, Social Workers etc. Oral Health Providers\, Denta
 l Hygenists\, and Dental Assistants) \n\n\n\n\nAgenda\nPresentations from
  your health center peers\, state agencies\, non-profits\, and consultants
  will be interactive\, creative\, dynamic\, and feature best practices aro
 und population health management that include the people\, processes and t

  FIDSA\, CMO at MDHHS presenting on state priorities that Michigan CHCs ne
 ed to be aware of regarding current and emerging infectious diseases\n 	MD
 HHS diabetes unit will provide an overview of the MIDPP Medicaid benefit\,
  in addition to a discussion about the populations now eligible for diabet
 es technology coverage\n 	Clinical leadership from 2 Michigan CHCs will pr
 ovide an overview of a standardized HTN clinical pathway\, soon to be pilo
 ted across the MPCA network\n 	Elevation Health Partners and Michigan CHC 
 panelists will share best practices to effectively improve performance on 
 MCHN VBC priorities\n 	Day 2 keynote speaker: Jennifer Torres\, PhD\, from
  MPHI presenting on the role of health equity in preventing and managing c
 hronic disease\n 	National Kidney Foundation of Michigan (NKFM) &amp\; MPC
 A\, along with Michigan CHCs will share tools and best practices implement
 ed to improve chronic kidney disease screening\, diagnosis\, and managemen
 t\n 	American Heart Association presentation on evidence-based resources\,
  clinical to community linkages\, and SMBP programs to improve BP control\
