SUMMARY:2024 Clinical Services and Quality Conference
DESCRIPTION:  \n\nOverview\nDON'T MISS IT!!!\nRegistration is now open for
  the 2024 MPCA Clinical Services and Quality Conference! This two-day in
 -person event will take place at the Ann Arbor Marriott Ypsilanti at Eagle
  Crest in Ypsilanti\, Michigan on May 15 and 16\, 2024. The conference is 
 aimed at Michigan Community Health Centers\, and it will concentrate on in
 tegrative strategies that promote patient-centered\, cost-effective\, and 
 quality care.  \n\nDay One will open will a keynote session provided by P
 atricia Braun MD\, MPH\, FAAP. Dr. Braun is a Professor of Pediatrics\, Fa
 mily Medicine\, Public Health and Dental Medicine at the University of Col
 orado Anschutz School of Medicine\, Colorado School of Public Health\, and
  University of Colorado School of Dental Medicine. She is a practicing ped
 iatrician at Denver Health and Hospital (a large federally qualified healt
 h center) and an oral health disparity researcher at the University of Col
 orado Center for Native Oral Health Research and Adult and Children’s Co
 nsortium for Research and Dissemination Science. \n\nThe remainder of Day
  One will center on some of the people\, processes and programs that suppo
 rt improving access to care for vulnerable populations. These sessions wil
 l showcase how the role and scope of a clinical pharmacist and dental the
 rapist can be utilized to increase touch points with patients. Presentatio
 ns will also highlight the patient benefits of receiving care in a deliver
 y setting that has embedded the Collaborative Care Model and that provid
 es the national Diabetes prevention education internally. We will also ad
 dress sexual health stigma by providing equitable and inclusive approaches
  that build care team capacity and patient access to STI screening.   \
 n\nDay Two will continue the discussion around capacity building and tools
  to ensure chronic disease\, oral health and mental health screenings are 
 accessible to everyone. Then in the afternoon we will spend the remainder 
 of our time together discussing care management and care coordination stra
 tegies that support decreasing avoidable utilization. The final session wi
 ll be formatted as more of a workshop. We will engage in some process mapp
 ing around how best to incorporate the role of a CHW within an integrative
  model of care.  \n\nThe target audience for this conference includes: \

 CMOs\, Clinic Directors) \n 	Dental Directors and Providers  \n 	Quali
 ty Improvement Staff  \n 	Clinical and Non-clinical Support Staff (Care
  Managers. BH Specialists\, Medical Assistants\, CHWs\, Nurses\, Social Wo
 rkers etc.)  \n\n\n\n\nMPCA In-Person Event Health and Safety Policy\nP

 his in-person meeting.\n\n\n\nContinuing Education\nThe conference has bee
 n approved for the following types of continuing education credits.\n\nPhy
 sicians | 12.25 CMEs\nThe AAFP has reviewed 2024 MPCA Clinical Services an
 d Quality Conference and deemed it acceptable for up to 12.25 Live AAFP El
 ective credit(s). Term of Approval is from 05/15/2024 to 05/16/2024. Physi
 cians should claim only the credit commensurate with the extent of their p
 articipation in the activity.\n\nNursing | 12.25 CEUs\nThis nursing contin
 uing professional development activity was approved by the Wisconsin Nurse
 s Association\, an accredited approver by the American Nurses Credentialin

 f Michigan Social Work Continuing Education Collaborative (CEC)\, an accre
 dited approver by the National Association of Social Workers (NASW).\n\nCo
