Michigan Primary Care Association supports the integration of behavioral health and primary health care, which has been shown to enhance access to services, improve quality of care, and decrease health expenditures.


The Behavioral Health Integration Map below displays current statewide efforts to develop and implement integrated care projects. Projects at 130 different sites are documented here, each unique in its strategy and scope. Organizations represented include Health Centers, Community Mental Health Centers, School-Based Health Centers, Rural Health Clinics, Public Health Departments, and Substance Abuse Agencies.


Use the below map to learn about integrated care models that are being pioneered across Michigan. The color-coded markers represent the type and level of collaboration, as shown in the Collaboration Continuum Legend. Projects range on a continuum from minimal, referral-based collaboration to a fully integrated provider team.
