Michigan Primary Care Association is pleased to announce a call for presentations for Improving Patient Health - It Takes the Whole Team, Fall Clinical Conference. We invite you to consider submitting an application to share information on models of care, successful patient interventions, coordination of services across disciplines internally or with external partners, or other innovative approaches to providing coordinated comprehensive care.

Conference Attendees

This conference will include topics for most clinical services within a Health Center and other community safety net providers. MPCA welcomes all organizations to attend and is offering a comprehensive agenda to provide educational opportunities and networking for oral health providers, behavioral health professionals, pharmacy staff, and primary care clinicians, and enabling services staff.

General Sessions

Presentations will have relevance to all health professionals. General sessions will inform and inspire.

Breakout Session Topics of Interest

Presentations highlighting primary care, behavioral health (including mental health and substance abuse), oral health, and pharmacy for all populations, multidisciplinary presentations (integrating at least two disciplines) highlighting interdisciplinary care for special populations; programs highlighting community partnerships to create a team-based, model of care for the patient, data-driven population management initiatives, patient care planning, self-care management and support services, referral tracking and follow-up models, and continuous quality improvement projects across an organization.


Other topics for consideration include: comprehensive care for all ages, dual eligible populations, quality management plans, coordinated care plans, PCMH and Health Home models, comprehensive chronic disease management, health information technology, diagnosing and treating mental health conditions, workforce support, patient information sharing (oral, pharmacy, behavioral health, and primary care), and paradigms for patient-engagement and patient-partnership.

Submission of Presentation Application

The information provided in the application below will help the planning committee to assure appropriateness of content and to develop program description and learning objectives. Description and learning objectives may be edited and formatted for inclusion in conference brochure. Timely responses to a request for all materials will be required as we will be submitting for Social Work CEUs, Provider CEs, as well the potential for other continuing education credits.

The conference planning committee is looking for presentations to document specific strategies that may be replicated within other communities and/or health care organizations.


Criteria for evaluating the presentations will include: 1) the topic relevance to a comprehensive approach to treating and caring for patients; 2) support of cross discipline coordination of care and/or systems; 3) ability to replicate or use information/design in Health Centers and other safety net provider settings

Format for presentation: The presentation should be prepared for a 60-minute session and allow time for questions and answers. MPCA will provide a typical room set-up including audio visual equipment and other supplies as needed.

Process for submitting and presentation:

• Complete the form below and save it as a document.
• Email to a resume or bio for each presenter; NO MORE THAN TWO PRESENTERS will be accepted per submission
• Each presenter accepted will receive one complimentary conference registration.
• Travel reimbursement will not be issued to any presenters for breakout sessions
• Return via email to by September 4, 2015.
