Presenter Bios for Learning the Principles of PCMH Among Peers

Susan Griffin, MS, has 25 years of health care experience and leadership with managed health care plans and health care clinics. Her expertise includes compliance, accreditation, quality improvement, disease management, credentialing, customer/member services, appeals and grievances, provider/member database maintenance, claims, provider relations, and all types of contract negotiations including fee for service, full risk, partial risk with and without pools, per diem, per case, and Medicare and Medicaid based reimbursement structures. Her current initiatives include the creation and operation of Patient Centered Medical Homes.

She operates her own health care consulting firm serving HMO’s, MA-PD’s, PSN’s, Health Care Clinics, Physician Groups, Urgent Care Centers, Accountable Care Organizations, Ambulatory Surgery Centers, and Community Health Centers/ Federally Qualified Health Center’s.

Susan has been an AAAHC surveyor since 2006 and is a member of the Standards and Survey Procedure Committee.




Mona Sweeney, RN BSN, is the Assistant Director for accreditation services with the Accreditation Association for Ambulatory Healthcare (AAAHC) since 2012. She is actively involved in the development, education, implementation and promotion of the primary care and medical home programs, standards and survey process. As project manager for HRSA’s Accreditation Initiative contract, she works with FQHC’s to achieve medical home accreditation. Previous roles included Vice President of Business Development at BrightStar Care, visiting Fellow at Northwick Park Hospital in the UK working with Barbara Amsel MD and the Immunology, Rheumatology Clinician at Children’s Memorial Hospital in Chicago where she held both clinical and research responsibilities.
