Job Summary: The Physician systematically assesses patient’s health status, arrives at appropriate diagnosis(es), and implements care plans for effective clinical outcomes. ICH is a team-based primary care center with integrated onsite behavioral health, Psychiatry, and RN case management. Laboratory and Pharmacy Services are also located on site. The Physician will work collaboratively to create and maintain a culture of excellence and dedication to providing compassionate and high-quality health care to the community. ICH is a Monday- Friday outpatient only practice. ICH follows the Patient Centered Medical Home modal of care.
Qualifications:
Required
- Current medical licensure in the State of
- Board certificated or board eligible in
- Drug Enforcement Administration (DEA)
- Current BLS certification
