Pharmacist

Position Description: Part-time; hourly; approximately 30 hours per week.

We are looking for some great staff who enjoy helping patients. You should love making complicated situations simple and painless and have a passion for our mission. You’ll be responsible for providing excellent patient care for Health Park Pharmacy and Isabella Citizens for Health. You’ll help us interact with patients and other members of the healthcare team via phone, messaging, and in person. You will work scheduled shifts with the expectation of being somewhat flexible.

We have a strong preference for a Pharmacist who has experience with 340B, and wants to make the pharmacy (and our community) a better place. Come and see how pharmacy was meant to be!

The Pharmacist is responsible for screening prescribing provider orders to ensure the correct medications are given to individual patients of all ages, assuring the most correct drug and dose is prescribed, dispensed, administered and properly monitored. The pharmacist screens orders for proper drug/dosage, drug interaction, adverse reactions, allergies, and

contraindications. Also, researches and answers drug information questions and provides various clinical pharmacy services. The pharmacist is a member of the multi-disciplinary care team.

Essential Functions and Responsibilities:

  • Accepts, validates, interprets, and processes prescription
  • Promotes the mission and philosophy of providing quality healthcare and related services to the medically under-served.
  • Ensures pharmacy is compliant with best practices for a 340B
  • Ensures quality controls for pharmacy practice by following standard operating procedures and all legal requirements set by the DEA and state pharmacy
  • Ensures all actions, job performance, personal conduct and communications represent the organization in a highly professional manner.
  • Maintains a commitment to the achievement of quality health care services by maintaining patient rights, safety, privacy and confidentiality.
  • Initiates consultation with prescribing providers and patients to discuss appropriate drug therapies and offers education and information.
  • Oversees support personnel in a team
  • Conducts Drug Utilization Review and provides necessary clinical
  • Thoroughly conducts Comprehensive Medication Reviews (CMRs), clinical interventions, patient monitoring, coaching, and follow-up activities. Documents activities appropriately in MTM platform for reimbursement.
  • Responsible for Formulary Management, including Therapeutic Interchange or Substitution, and Coverage Review Programs such as prior authorization.

Requirements:

  • Bachelor Degree in Pharmacy or Doctor of Pharmacy and current pharmacist and controlled substance license in the State of Michigan.
  • A minimum of two years of

Preferred

  • 340B exper
  • Completion of an ACPE accredited immunization training
  • Current BLS/CPR
  • Experience in retail/community practice
