The board of directors of the Michigan Health Endowment Fund has awarded a $5 million pilot grant to Michigan Primary Care Association to support its implementation of interdisciplinary care teams aimed at improving the quality of health care in Michigan while reducing costs.

“We are tremendously appreciative of this funding that will enable us to hire, train, and integrate more than 30 community health workers into collaborative care teams in the first year at 16 Health Centers across the state,” said Kim Sibilsky, Chief Executive Officer, Michigan Primary Care Association (MPCA).

MPCA is the statewide association representing Michigan Health Centers and other community-based providers. Michigan Health Centers provide quality, affordable primary and preventive health care to more than 600,000 Michigan residents at over 240 delivery sites. Their doors are open to all, regardless of insurance status or income level, and their comprehensive services include medical, dental, and behavioral health care, as well as translation, insurance enrollment assistance, and other services that overcome barriers to accessing care.

Community health workers are front-line public health workers critical to providing one-on-one health education and care coordination, especially for vulnerable populations within their own communities.

“By serving as liaisons between community members and health and human services, community health workers greatly increase access to care, improve health and quality of life, and significantly build community capacity to address health issues in a cost-effective way. The Michigan Health Endowment Fund’s investment in expanding the reach of community health workers provides an unprecedented opportunity to improve the health of our neighbors, our communities, and our state. MPCA and Michigan Health Centers are honored to be among the pilot grantees working with the Fund to build a healthier Michigan,” said Sibilsky.
