POSITION SUMMARY:
Reporting to the Supervisor, Behavioral Health the full time, exempt Behavioral Health Clinician is an essential member of a high performing healthcare team. The Behavioral Health Clinician (BHC) Child and Adolescent Health Center (CAHC) will provide clinical behavioral health services to assure a professional, skilled program of care and treatment is administered. In addition, the BHC will uphold the health center’s mission and quality standards which result in better patient health outcomes, overall satisfaction, and stronger operational and fiscal capacity of the health center. The BHC will adhere to Honor Community Health (HCH) policies and procedures while carrying out and monitoring a wide range of clinical processes. The BHC will maintain strong internal and external relationships, serving as liaison on behalf of HCH’s centers. The BHC works collaboratively to create and maintain a culture of excellence and dedication to providing compassionate and high-quality behavioral health care to all. Productivity expectation of a trained CAHC BHC is an average of 7 or more behavioral health consultations/counseling per clinical day.
ESSENTIAL FUNCTIONS:
- Assessment
- Conducts assessment of the presenting issue that is descriptive considering frequency, intensity, and environmental context in which the issues occur.
- Diagnosis formulation is clear and includes significant etiological factors, identify key difficulties, includes history of symptoms, onset, and precipitating factors.
- Identify the level of care that is appropriate for the individual’s presenting issues.
- Considers necessary interventions based on evidence-based practices.
- When necessary, clinician will evaluate for risk of harm, current and past functional levels, support system within the individual’s environment, and explore co-morbid/co-occurring conditions.
- Limited use of screening and assessment as appropriate. Individual, couples, and/ or family counseling with patients, including-
- Functional and strength-based assessment and diagnosis,
- Psychoeducation for patients and their support systems,
- Rapid Assessment for Adolescent Preventive Services (RAAPS) risk assessment,
- Medication adherence counseling and disease self-management counseling,
- Motivational Interviewing to develop behavioral strategies aimed at symptom reduction,
- Brief problem solving and solutions-focused interventions aimed at modifying negative thinking and promoting self-efficacy.
- Treatment
- The goals are in the individual/guardian/family’s words and intentions based on individual’s understanding of the assessment, findings, and recommendations of all his/her/their support systems. The treatment is based in
- Goals and objectives are written as attainable, measurable, have observable outcomes, realistic, written in positive terms, build upon abilities/strengths, preferences and needs, and they embody hope/alternatives to a current situation.
- Develops treatment plans and reviews as needed/indicated, including the following:
- Conduct pregnancy follow-up counseling regarding personal options, psychosocial stressors, and lifestyle risks, among others.
- Develop and facilitate support groups as necessary.
- Meet regularly with other school-based health center staff and occasionally with school-based health center advisory group,
- Periodically attend state School-Based health center workshops and other appropriate continuing education functions,
- Maintain professional relationship with school faculty and staff and referral resources.
- Provide counseling sessions for patients, parents, and students.
- Crisis Intervention – Be immediately available to the health center staff during working hours in the event of a psychiatric emergency. Participate as a member of the health center staff in the event of any other kind of emergency.
- Assists in the coordination of and delivery of the Youth Advisory Council and Community Advisory Council.
- Coordination of Care
- Accepts referrals from the medical provider regarding patients’/student psychological / behavioral needs.
- Conduct health education sessions as needed to students inside classrooms and or pull-out groups.
- Provide consultation to and coordinate care of patients with health center primary care staff. Identify, refer, and advocate for patients needing specialty behavioral health service, and other services as needed. Assist in coordination of services between school agencies, treatment programs such as Children’s Village, school district, and other mental health agencies.
- When needed the BHC will consultation with PCPs, teacher, school counselor, and other mental health professionals to enhance understanding of the patient, provide decision support for treatment planning and assist in the implementation and monitoring of biopsychosocial treatment plans.
- Documentation
- Charting – Complete all charting as needed and required by contacted funding sources: including assessments, progress notes, and billing. Charting should be completed in a timely fashion, no later than 72 hours.
- With appropriate medical release, works with school based social workers, school counselors, and clinical staff to develop a plan of care for each patient.
- Documentation of progress reflects intervention, skills used to cope/adapt/respond/problem solve. Reinforce new behaviors and strengths. Identify specific skills that are taught/modeled/practiced. The Intervention elements of the progress note shall describe the following: clinician’s interventions, clinician’s assessment, including a risk assessment when applicable, document advice/recommendations given to individual and family, response to the interventions and progress or lack of it towards goals and next steps.
- Quality Improvement
- Participate in local/national behavior health trainings and conferences.
- Participates in HRSA and MDHHS/CAHC site review prep.
- Assist in provider training and education in the primary care behavioral health model.
- Facilitates and coordinates care of patients to ensure quality outcomes. Participate in quality improvement activities and actively participate in health center meetings related to integrated care.
- Community Outreach/Collaborations
- Develop and foster relationships with various community partners including but not limited to Community Mental Health care provider agencies, local health departments, local hospital networks, etc.
- Collaborate with the CAHC team to perform community outreach activities such as but not limited to the following: present at a school principals/teacher staff meeting, attend community health fairs, parent/teacher conferences, school district events, school district orientations, etc.
- Assist in the development and implementation of community advisory councils and youth advisory councils.
- Utilize MS Forms to track health education and Medicaid outreach activities.
- Provide mental health education if needed or requested to students inside the classroom or an assembly based on the needs assessment of the school district.
- Provide mental health education to teachers, admin, and school officials as needed or requested.

QUALIFICATIONS:
PROFESSIONAL/TECHNICAL KNOWLEDGE, SKILLS & ABILITIES
- Master’s degree in social work, or related field.
- 2 years’ experience with mental health.
- Current license to practice in Michigan as a Licensed Clinical Social Worker, Limited License Psychologist, License Professional Counselor, Doctoral degree in psychology.
- Knowledge of primary care and healthcare integration is strongly recommended.
- Must maintain BLS Certification.
- CAABC/substance use experience preferred.

WORKING CONDITIONS: Works in a high-paced work environment with multiple deadlines. Interaction with angry and hostile patients may occur. Regular work and deadlines impose strain on a regular basis. Work non-traditional hours to meet the organization’s needs and accomplish duties of position which may include evenings and weekends. Local travel between sites may occur.
