Overview

Please mark your calendars for November 2nd and 3rd, 2023.  The conference will take place at the Crowne Plaza West Lansing.  This two-day in-person event will focus on initiatives, tools and best practices that support equitable and sustainable population health management programs in Michigan Community Health Centers.

• Quality Improvement Staff 
• Clinical and Non-clinical Support Staff (Care Managers. Medical Assistants, CHWs, Nurses, Social Workers etc. Oral Health Providers, Dental Hygenists, and Dental Assistants) 

Agenda

Presentations from your health center peers, state agencies, non-profits, and consultants will be interactive, creative, dynamic, and feature best practices around population health management that include the people, processes and technology needed for success. Some of the confirmed sessions, with more to come, include:

• Day 1 keynote speaker: Dr. Bagdasarian, MD, MPH, FIDSA, CMO at MDHHS presenting on state priorities that Michigan CHCs need to be aware of regarding current and emerging infectious diseases
• MDHHS diabetes unit will provide an overview of the MIDPP Medicaid benefit, in addition to a discussion about the populations now eligible for diabetes technology coverage
• Clinical leadership from 2 Michigan CHCs will provide an overview of a standardized HTN clinical pathway, soon to be piloted across the MPCA network
• Elevation Health Partners and Michigan CHC panelists will share best practices to effectively improve performance on MCHN VBC priorities
• Day 2 keynote speaker: Jennifer Torres, PhD, from MPHI presenting on the role of health equity in preventing and managing chronic disease
• National Kidney Foundation of Michigan (NKFM) & MPCA, along with Michigan CHCs will share tools and best practices implemented to improve chronic kidney disease screening, diagnosis, and management
• American Heart Association presentation on evidence-based resources, clinical to community linkages, and SMBP programs to improve BP control

Continuing Education

CME Elective Credits – Up to 10
The AAFP has reviewed The 2023 MPCA Fall Clinical Conference: Navigating  Population Health Management and deemed it accepteble for up to 10.00 Live AAFP Elective credits.

Nursing Contact Hours – Up to 9
This nursing continuing professional development activity was approved by the Wisconsin Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation.  

Exhibitor and Sponsorship Opportunities – Thursday, November 2

If you are interested in exhibiting/sponsoring this event, we will have a limited number of exhibit tables available on Thursday, November 2. The cost includes an 8-foot skirted table, chair, breakfast, lunch, and afternoon snack and beverages throughout the day.
