Enabling Services and Social Determinants of Health

Overview

Health centers provide more than cost-effective primary and preventive care — they also connect patients to health coverage and resources such as food or housing assistance and transportation and translation services. These enabling services, or non-clinical patient services that enable individuals to access health care and improve health outcomes, allow Michigan health center staff to help patients and communities address health barriers and Social Determinants of Health (SDOH), or the non-medical factors that influence health outcomes. MPCA supports enabling services staff, including Community Health Workers (CHWs), peer educators, and Outreach & Enrollment (O&E) specialists, with technical assistance, training, network communications, resource development and best practices sharing.
