Michigan Quality Improvement Network (MQIN)

The Michigan Quality Improvement Network (MQIN) is the Health Center Controlled Network (HCCN) for Michigan. HCCNs are networks of health centers that work together to strengthen and leverage health information technology (IT) to improve health centers’ operational and clinical practices that result in better health outcomes for the communities they serve.  Established in 2012, MQIN is responsible for providing support to Michigan’s community health centers to provide the best care for their patients.

MQIN hosts a quarterly webinar during the third month of each quarter. Check mpca.net/events for registration.

MPCA has partnered with Luma Health, a patient outreach and engagement platform, to support community health centers through the use of text messaging (SMS), email, and voice communication tools. These tools enable health centers to engage patients proactively, improve outreach efforts, and help close gaps in care.

Luma Health is integrated with Azara, allowing health centers to export patient lists directly from Azara reports into Luma. This integration supports targeted outreach based on care gaps, recalls, and other population health initiatives, helping organizations improve patient engagement and care coordination.

miEpic Collaborative EHR

Michigan Primary Care Association (MPCA), in partnership with Health Choice Network (HCN), is bringing miEpic to health centers within the Michigan community. miEpic is built specifically for Federally Qualified Health Centers (FQHCs) and leverages HCN’s nationally recognized Epic model, already proven in health centers across the country.

Privacy & Cybersecurity

Federally qualified health centers (FQHCs) handle a tremendous amount of sensitive and confidential health information from patients, including electronic health records and other digital data. As such, FQHCs have a crucial role in safeguarding their patients’ data and ensuring privacy and security at all times. Cybersecurity plays a crucial role in this process. The risks of data breaches, cyberattacks, and other digital threats are real, and FQHCs must prioritize cybersecurity measures to protect their patients’ data.

To ensure the safety of patient information, FQHCs should conduct regular audits and risk assessments to identify vulnerabilities in their systems and infrastructure and proactively address them. Staff education should be provided on patient privacy and data security policies. Through these steps and many more, FQHCs must work toward hardening their security posture through a multifaceted approach. MPCA (VirtuAlly) is here to help. Uniform Data System

Each year, Federally Qualified Health Centers (FQHCs) and look-alikes are required to report data to the Health Resources and Services Administration (HRSA). Michigan Primary Care Association supports health centers with tools, guidance and best practices for Uniform Data System (UDS) reporting.

Virtual Care

Virtual care encompasses the various ways healthcare providers can interact and engage with patients remotely. Common examples include telehealth and remote patient monitoring (RPM). Telehealth leverages information and communication technologies to enable providers to deliver healthcare services virtually, improving access to care and creating a more convenient, patient-centered experience for individuals who face geographic, transportation, or other access barriers.

MPCA supports its member health centers with the procurement, implementation, optimization, and sustainability of virtual care technologies through strategic partnerships, funding opportunities, and technical assistance.

Remote Patient Monitoring (RPM): MPCA previously secured grant funding and established strategic partnerships to expand RPM and virtual care services across Michigan’s community health centers. Through this previous FCC-funded initiative, nine health centers received RPM equipment and services, including Bluetooth-enabled blood pressure monitors, weight scales, and pulse oximeters, supporting enhanced patient monitoring and chronic disease management.

Today, MPCA partners with CAREMINDr, an innovative virtual care and patient engagement platform that extends beyond traditional RPM services. CAREMINDr supports chronic disease management, continuous patient engagement, remote monitoring, risk identification, and proactive care interventions. The platform offers solutions for conditions such as hypertension, diabetes, pregnancy, and behavioral health, enabling care teams to engage patients between visits, monitor health trends, and improve health outcomes through timely outreach and intervention. CAREMINDr combines patient-reported information, connected devices, and advanced analytics to support value-based care and population health initiatives.

Luma Health: MPCA’s Health Center Controlled Network has partnered with Luma Health, a patient outreach and engagement platform currently used by several member health centers. Luma Health enables organizations to communicate with patients through SMS text messaging, email, voice outreach, appointment reminders, and other engagement tools. Luma Health also offers telehealth capabilities and integrates with Azara to support targeted outreach based on care gaps, recalls, and population health initiatives.
