Overview

The “Introduction to Team-Based Care” course is designed to provide learners with a better understanding of how to work in a multidisciplinary care team and collaborate with patients effectively. This course is open to all care team members (nurses, MAs, CHWs, LMSW, Pharmacists) who wish to gain foundational knowledge in Team-Based Care.
The course will cover a range of topics related to Team-Based Care and equip learners with the necessary skills to work in a collaborative care team environment.
