Advanced Practice Provider (APP) – Primary Care

Located along the scenic shoreline of Lake Michigan, Hackley Community Care (HCC) is a Federally Qualified Health Center proudly serving Muskegon County and surrounding communities. Our dedicated team of 300+ staff members delivers integrated medical, dental, behavioral health, and support services to individuals and families most in need. We are currently seeking a compassionate and skilled Advanced Practice Provider (APP) – either a Nurse Practitioner or Physician Assistant – to join our Primary Care team. The ideal candidate is experienced in caring for patients across the lifespan, comfortable providing both routine and complex care, and passionate about improving access to quality healthcare for underserved populations.

What You’ll Do:
- Deliver primary care services across the lifespan, including wellness visits, sick care, follow-ups, and chronic disease management
- Provide walk-in care when needed
- Serve as an active member of a multidisciplinary care management team
- Prescribe medications and develop comprehensive treatment plans
- Communicate effectively with patients, families, and care teams
- Complete accurate and timely documentation of exams, treatments, referrals, and consultations
- Participate in preventive services and help ensure continuity of care
Qualifications:
- Graduation from an accredited Nurse Practitioner or Physician Assistant program
- Active and unrestricted license to practice as a PA or NP in the State of Michigan
- Current board certification in area of practice
- Current BLS/CPR certification
- Eligible for malpractice insurance coverage as a mid-level provider
- Prior experience in primary care preferred, but not required
