Upper Great Lakes Family Health (UGLFH) is seeking an Outpatient Clinical Therapist (LMSW, LLMSW or LPC) to serve as a integral member of the UGLFH care team and serving the patients of the Houghton Family Practice Clinic.

Responsibilities
- Provides a full array of behavioral health services to patients from children and adults, including but not limited to, and all within an integrated primary care clinic environment or via telehealth:
- screening and assessment
- care planning and coordination.
- interventions providing brief, focused prevention, treatment and recovery services.
- support with persistent illness.
- additional substance use disorder-related services may be provided if Practitioner has CAADC or MCBAP credentials.
- Experience maintaining confidential information and adherence to HIPAA and CFR 42 Part 2.
