Michigan Community Health Network (MCHN)
The Michigan Community Health Network (MCHN) is a clinically integrated network formed by Michigan community health centers for the purpose of group contracting with Michigan Medicaid Health Plans. It was formed in 2015 and is owned by the Michigan Primary Care Association and 39 of its member health centers. MCHN negotiates value-based agreements on behalf of its members for the purpose of improving overall outcomes of the patients it serves and sharing in the resulting cost savings.
MCHN seeks to support the financial sustainability of its member health centers by negotiating new incentive opportunities that reward health center efforts aimed at increasing access to care, improving HEDIS measure scores, and decreasing unnecessary utilization and total cost of care. MCHN supports these efforts through the creation of actionable data reporting, supporting technological solutions to improve outreach and engagement (Automated Text Messaging, Direct Mail), and supporting best practice workflows and process improvement activities. Payer partnerships exist to support the work of RN Care Managers and Community Health Workers.
MCHN continuously assesses new ways to support its member organizations. Most recently we are excited to announce the launching of a new insurance credentialing and enrollment service in 2025 which will include delegated credentialing agreements as well as additional services beyond delegation for our member health centers.
Agreements
MCHN has value-based agreements with seven Medicaid health plans and offers a Medicare REACH ACO and Medicare MSSP ACO through a partnership with Medical Home Network. The agreements cover over 200,000 lives and the efforts of the health centers have resulted in new value-based incentive earnings every year since its inception. These agreements provide additional funding to support the health center mission of serving the underserved as safety-net providers of care.
