
Overview
The “Introduction to Team-Based Care” course equips learners with the knowledge and skills to work collaboratively in a multidisciplinary setting, ensuring coordinated, safe, and patient-centered care. Open to all care team members (nurses, MAs, CHWs, LMSW, Pharmacists), the course covers foundational elements of Team-Based Care and Care Management, preparing participants to effectively integrate care management skills within health centers.
